In New Jersey, the medical record is the backbone of an injury claim. It shows what was wrong, when symptoms began, what treatment followed, and how the injury changed daily life. Insurers read these records line by line, and a claim without them is hard to prove.
The records matter at each step. Personal injury protection coverage in New Jersey pays medical expenses regardless of fault, and the insurer will want treatment notes and bills to support each payment.
To bring a claim for pain and suffering, an injured person may need a physician's certification of permanent injury, which must rest on objective clinical evidence such as imaging and exam findings.
Gaps and inconsistencies are what insurers look for. A delay before the first visit, missed appointments, or a symptom that appears in one note but not the next can be used to argue the injury was minor or came from something else.
It is important to describe every symptom to each provider, follow treatment plans, and keep copies of bills, prescriptions, and referrals to keep the record complete.
